Red Surprise Bags were added during Tibia's 10th Anniversary, when they were rarely dropped by strong monsters. These bags contain rarer items than Blue Surprise Bags, but these in their turn contain some unique festive items.
The red surprise bag can contain festive items and rarer items. The following items are obtainable by using a bag:
Fireworks Rocket
Cream Cake
Bar of Chocolate
Party Hat
Piggy Bank (semi-rare)
Violet Gem (rare)
Soul Orb (rare)
Red Gem (very rare)
Teddy Bear (very rare)
Demon Shield (very rare)
Panda Teddy (very rare)
Boots of Haste (very rare)
Dragon Scale Mail (very rare)
Royal Helmet (very rare)
Amulet of Loss (very rare)
Stuffed Dragon (very rare)
During Tibia's 15th Anniversary, the following approximate changes were made:
Cream Cake →

5 Small Diamonds
Violet Gem → 
15 Platinum Coins
Red Gem → 
Gold Ingot
Because they bear similarity in probability, they may even share exact loot tables save for those item changes. The obtained items have now been reverted back and Violet Gems for example can be obtained from 15th Anniversary Red Surprise Bags again.
During Tibia's 10th Anniversary, Red Surprise Bags may have also contained Red Balloons.

Can be obtained by skinning the Slain Mutated Pumpkin inside its reward room and by using Surprise Nests. Could also be obtained from Anniversary Remains during Tibia's 15th Anniversary.
