It is one of the pieces of Koshei's Ancient Amulet. Part of the Koshei the Deathless Quest. It can be found in a Stone Coffin on the third floor underground of the Dark Pyramid, in the far northeastern part of Darama (here). After delivering all four pieces of the amulet (and 5000 gp) to A Sweaty Cyclops he can give you the complete amulet after waiting for 24 hours in real-time.
