There are various kinds of leaders in Tibia, from royalty and political leaders to organisation heads and military leaders. Some rule entire continents and some rule just a few people.

Rulers

Rulers are the ones heading states, and usually have ancestors who were also royalty. Everyone in their entire kingdom is their subject, but this does not mean that all of them also agree with the way they are ruled. Most rulers have the ability to promote worthy adventurers.

Representatives, while not themselves heads of state, represent their rulers in the outposts or colonies where they are stationed. However, of all representatives only Grand Viziers can bestow honours such as promotions. This means that most of the time, you will have to visit the rulers of Tibia's main cities directly if you want to be promoted.
