Agony Damage is one of the types of Magical Damage. It is displayed in game as Brown Text.
Players are not able to use this damage type by themselves, however the library entries for Two-Headed Turtles state that they are immune to this type of damage and for Foam Stalkers state agony damage heals them. It is also confirmed that it is not possible to reduce taken agony damage with equipments or items.
The Abomination and Morshabaal are two bosses that deal Agony Damage. During the fight against The Brainstealer players receive Agony Damage from the curse applied to them during the fight. Also, in the fight against Magma Bubble, players can also take Agony Damage from the Fire Fields in the room if they walk over it without protection (gained through room mechanics), and in similar fashion during the In the Heat of the Fight task of the Primal Ordeal Quest.
It is possible to also receive agony damage in the Tibiadrome, depending on the active modifiers.
On April 18, 2023 burning walls in the Cobra Bastion have been changed to deal Agony Damage to players standing in them (Gaffir and Guard Captain Quaid spots).
As part of the Summer Update 2023 two new areas, Gloom Pillars and Darklight Core, introduce new mechanics that deal agony damage to players. In Gloom Pillars players absorb dark energy until a certain number of charges then take agony damage from a chain spell while in Darklight Core creatures put darkfields below characters similar to a fire field but that deals agony damage instead.

It used to be possible for Elite Knights to reduce agony damage with Protector however that's not the case anymore since the Summer Update 2022 implementation. Additionally, the same update improved the display of agony damage on input analysers that were previously displaying this type of damage as Physical Damage.
